Hi there, please fill out this form if your student has a 504 or IEP and will need testing accommodations for the SAT or ACT. Depending on the accommodation requested, additional documentation may be required. Please have the 504 Plan or IEP ready.

If the student has not submitted a form yet, please have the student complete the ACT Consent to Release Information Form or the College Board Consent for Accommodations Request Form (for AP and SAT exams) before continuing.

College Board assessments do not allow self-pacing therefore students will be required to sit for the entire duration of the extended time requested for each test or section type.
